Congratulations on the birth of your child! The birth of a child is a joyous event for families--including the Catholic Church family. Through the sacrament of baptism, we become members of Christ's Church and the Body of Christ. Below you will find all the information to begin the process for your child to receive the sacrament of baptism.
If you are not registered with the parish, please complete our online registration form. This form should be completed if you are a visitor and attend another parish.

Below are the Church's requirements for the role of godparents/sponsors.:
The Church requires only one sponsor/godparent. This person must be at least 16 yrs of age and a Catholic who has been confirmed and practices the faith (i.e., one who attends Sunday Mass on a regular basis.) If married, the person's marriage needs to be one that was celebrated in the Catholic Church.
Proof of the Catholic sponsor/godparent’s confirmation is required. Please provide us with a copy of the LONG FORM of the baptism certificate. You can obtain this by contacting either the church where the confirmation took place or the church of baptism. A copy of the long form baptism certificate (front and back) can be emailed to the baptism coordinator at the email address below.
Along with the confirmed Catholic, you may have a baptized Protestant who agrees to help you raise the child in the Catholic faith. This person is recognized as a Christian witness.
If you have two sponsors, then there needs to be one of each gender.
Sponsors/godparents are required to attend a Baptism Preparation class if they have not previously attended one in the past two years. The class can be taken at St. Theresa’s or at another Catholic parish if that is more convenient.
St. Theresa’s Baptism Preparation classes are held on the first Sunday of the month at 12:30 PM in the parish center.
